In order to induce the male breasts to produce milk, a oestradiol therapy in combination with appropriate doses of drugs such as the commonly used gastroprotectant domperidone. The 'therapy' also works for women who have difficulty in breastfeeding (Newman-Goldfarb protocol). Galactorrhoea (milk production from the mammary glands) is a side effect of domperidone. This effect is explained by its antagonistic action on dopamine, a neurotransmitter that inhibits the secretion of prolactin, the hormone that initiates lactation, at the pituitary level.

In pregnancy and breast-feeding women, therefore, the drug should only be taken in in cases of absolute necessity and under direct medical supervision. Domperidone may also cause undesirable effects on the heart of the breastfed infant. Not to mention oestradiol.
